Apply for service by completing the Information Request Form or print the Propane Application and mail to Polk-Burnett today.
What you need to know before completing the application:
Propane is a very efficient fuel, however it is also very volatile. Polk-Burnett doesn't want you to jeopardize you or your family's safety, so we highly recommend that you use a competent licensed contractor to install, test and service your propane gas appliances. We also recommend the installation and maintenance of gas detectors on every level of your home.
Prior to scheduling the actual tank set, we need:
- Completed propane application.
- A site inspection by Polk-Burnett's propane personnel. This will help determine tank location, size of structure being served, potential delivery or driveway difficulties and other safety concerns.
- If no problems are encountered with the site inspection and your request is for new construction, a call to Digger's Hotline may be necessary to mark underground lines, 800-242-8511, 811 or www.diggershotline.com.
A Credit and Estimated Usage Report for the past two years from your most current propane supplier may be required if you're switching from a different propane supplier to Polk-Burnett.
There is no charge to switch to Polk-Burnett if your system is leak free and code compliant.
Tank set includes trenching the piping at 12 inches or deeper, pressure testing the new piping and leak testing the system in compliance with NFPA code. If propane appliances are not installed or hooked up at the time of the leak test, Polk-Burnett can only test up to the second stage regulator on the home. After your appliances and interior piping are installed, your licensed heating contractor is responsible for hooking up each of your appliances to the second stage regulator. The heating contractor is also responsible for additional leak/pressure testing. Please make sure the test is completed and supply us with a copy. Otherwise call Polk-Burnett Propane Services to schedule a complete leak test for you, 800-421-0283.
